Job Description

Pay rate: $29.00 - $31.00 per hour, DOE. Sign-on bonus of $1,000.00 upon hire . Generous benefits package including medical, dental, and vision insurance, paid time off, childcare discount. Mileage reimbursement. 40 hours/week. Schedule Mondays through Fridays. hybrid schedule possible in accordance with NMCS and manager approval. 

The Bilingual Housing and Benefits Navigator serves to connect NMCS Case Management program participants to resources in the community. This position directly supports a team of case managers in leading efforts to assist our community in finding wellness, dignity, housing, and public benefit resources. Provide screening, assessment, and some light touch case management, and life skills education to support youth, adults, and families at NMCS and in the community. In addition, provide care coordination efforts including meeting with participants and collaborators in an office and outreach setting, assisting with applying for housing, communicating with landlords, completing rental assistance applications, gathering documents, and other activities that support continuity of care. Assist with CalAIM and DHCS initiative as necessary by working collaboratively with internal and external partners to ensure that program participants are connected with all available resources.

Essential Functions:

  • Provides screening/interviews with program participants seeking case management services through NMCS as part of the intake process, as well as periodic follow ups. Provides specific screening for CalAIM and other program eligibility.
  • Works collaboratively with NMCS case managers and their supervisors.
  • Assists program participants in navigating the safety net system of care, as well as providing appropriate linkages to various county and community agencies, based on need and availability.
  • Create strong landlord/housing relationships locally and create landlord database.
  • Develop strong collaborative relationships and participate in agency meetings, inter-agency meetings, case consultation, NMCS assigned trainings, and other activities as needed.
  • Train staff and support participant and agency efforts to increase access to public benefits such as Medi-Cal, CalWORKS, CalFRESH, CAPI, SSI/SSDI/SSA, SDI, and General relief.
  • Documents all service contacts in a timely manner and maintains case records in accordance with agency standards and requirements using NMCS’ electronic records system (Apricot Social Solutions) and other data systems. Ensure that all paperwork and charting requirements/reports are completed correctly and submitted on time.
  • Creates referral linkages with providers, community organizations and identify providers to implement community resources, health, and wellness events and/or educational programs that address barriers to care, resources and enhance knowledge, and ultimately impact healthier individual and family outcomes.
  • Ensures that all records strictly comply with policies, regulations, and HIPAA procedures.
  • Assists program participants with the completion of relevant paperwork with professionalism, diplomacy and advocacy.
  • Provides culturally-sensitive, trauma-informed services that acknowledges, respects, and integrates participants’ and families' cultural values, beliefs, and practices.
  • Be a strong ambassador for the agency including knowing and adhering to the agency’s mission, values, and programs; referring program participants; and engaging volunteers and donors when possible.
  • Job may include walking up and down stairs, standing, lifting and moving small items of up to 25 lbs.

Required Qualifications:

  • Bilingual fluency in English and Spanish, both verbal and written
  • Bachelor’s degree in Social Work, Human Services, Addiction Studies, Psychology, or related field OR 1-2 years of behavioral health, public benefit enrollment, and/or case management experience
  • Excellent customer service with a program participant focus; strong verbal and written communication skills
  • Proficient in Microsoft Office Suite
  • Ability to maintain professional boundaries with participants while building trust and respect using trauma-informed and culturally responsive practices.
  • Valid Class C driver’s license and insurability at non-risk rates; personal vehicle to meet participants in the community.

  • Bicultural
  • Significant experience working with immigrant communities; experience working with Black, Indigenous People of Color (BIPOC).
  • Experience working with electronic health records and Medi-Cal.

Conditions of Hire:

  • Fully vaccinated against COVID-19 unless granted a medical or religious exemption.

  • Criminal record clearance or exemption from California Department of Social Services. All convictions other than minor traffic violations require an exemption, including convictions that have been expunged.
