In this role, you would be responsible for developing and implementing an individualized Plan of Care for each resident in the Health Care Center, and completing the MDS accurately and in compliance with all applicable regulations. The MDS/Care Plan Coordinator is actively involved in the Interdisciplinary Care Team, and attends all resident care conferences. As part of the nursing administration team, she/he shares on-call responsibilities. The MDS/Care Plan Coordinator should reflect and promote the philosophy of care of the Lakewood Nursing department and is delegated the administrative authority, responsibility and accountability to carry out assigned duties. JOB QUALIFICATIONS:

  • A Registered Nurse and graduate of an accredited school of nursing. Must hold an active license and be in good standing with the State of Virginia.
  • Previous experience of one (1) to three (3) years in a Skilled nursing facility is recommended.
  • Must possess excellent oral and written communication skills.
  • Must be proficient on the use of computer systems
  • Must have the ability to communicate positively with coworkers, residents, family members, visitors, government agencies, and the medical community.
  • Must have excellent assessment skills, and be able to use good nursing judgement to develop and update Care Plans that meet the needs of individual residents.
  • Knowledgeable about all applicable federal, state and local regulations pertaining to long-term care and the MDS process.
  • Knowledgeable about geriatric care and the aging process is a plus.

DUTIES:

  • Complete resident assessments within the time frame required under current regulations.
  • Participate as a member of the Interdisciplinary Team in Care Plan conferences to present the care given to residents, and address resident/family concerns. Develop interventions and see that they are included in the resident centered Care Plan.
  • Conduct random audits for monthly summaries and care plans performed by nurses for timeliness and accuracy.
  • Review resident medical records to ensure complete documentation.
  • Notify the Director of Nursing/Administrator of Healthcare, of any resident care problems requiring his/her attention.
  • Provide clinical support to the nursing staff as needed to ensure quality resident care.
  • Perform in a manner that communicates personal responsibility and self-discipline. Hold self accountable for own actions to include relationships with others and performance.
  • Develop the MDS/Care Plan Schedule and notify members of the Interdisciplinary Care team.
  • Monitor for additional MDS’s needed (i.e. admissions, discharges, change in Hospice company, significant change in resident condition, etc.)
  • Keep the Director of Nursing, and Care Team members informed of information relating to resident care.
  • Complete and update Utilization Review meeting schedule including tracking of exhaust dates.
  • Work with Rehab Department to set ARD dates for Skilled residents.
  • Input ICD-10 diagnosis codes in to the resident medical record. Acts as resource for nurses on the ICD-10 coding process.
  • Share on-call responsibility. May provide direct care when necessary.
  • Participate in the review and development of department policy and procedures as needed.
  • Participate in survey and inspections made by government agencies – remains familiar with Department of Health and Federal regulations governing Lakewood, and OSHA regulations.
  • Represent the community in interactions with residents and families, staff, practitioners, Medical Director and others involved in providing care and services to residents utilizing effective communication.
  • Ensure resident confidentiality is upheld at all times (HIPAA).
  • Serve on, participate in, and attend various committees of the Community as required and as appointed by the Director of Nursing or Administrator of Healthcare.
  • Evaluate and implement recommendations from established committees as they pertain to Nursing Services.
  • Participate in Quality Improvement Programs to ensure quality excellence, reduce risk of liability, and enhance resident care and satisfaction.
  • Participate in the Interdisciplinary Care Team meetings.
  • Complete and update resident focused Care Plans according to regulatory guidelines.
  • Assure that Care Plan is initiated upon admission of new HC residents.
  • Complete the MDS in compliance with current regulations and guidelines.
  • Assure that other disciplines complete assigned sections of MDS within designated time frames per regulation.
