Job Description
Job Purpose:
Under the direct supervision of the Social Health Program Manager, the Mobile Health Unit Coordinator/Patient Navigator (MHUC/PN) functions in an integrated team as a front-line public health worker who breaks down Social Health barriers for Waterfall patients. The MHUC/PN is responsible for scheduling the mobile health unit for Waterfall Community Health Center (WCHC) within Coos County and surrounding areas. This will require extensive networking with community partners and leaders to schedule and host dental and medical visits, cancer screening and prevention, vaccine, and other outreach events. The MHUC/PN will also coordinate a patient panel to ensure equitable access to care for patients living outside the Coos Bay/North Bend municipality.
Duties/Responsibilities:
- Adhere to the spirit of Waterfall Community Health Center to remove barriers to care and our mission statement while performing assigned duties.

- Research and implement best practices to provide mobile health unit (MHU) quality patient care and serve as the communication link between MHU patients, clinical staff, and the integrated care program; provide outreach to assist patients in navigating the healthcare system and access to health services and resources.
- Routinely document all patient interactions in the Electronic Health Record (EHR).
- Facilitate data collection, aggregation, and display for all MHU activities.
- Attend monthly Quality Improvement (QI) Committee meetings and report data on MHU initiatives in collaboration with the rest of the Quality team.
- Work in collaboration with the Population Health Coordinator and Clinic Manager to identify patient populations that have difficulty accessing care and create a plan to provide care to these individuals. Identify patient groups that require proactive outreach and in-reach for preventive health maintenance and/or disease monitoring.
- Facilitate interdepartmental collaborations to develop, implement and carry out MHU events to improve chronic disease management, patient screenings, and closing care gaps for identified patients.
- Actively engage with community partners and leaders from surrounding areas to build and maintain rapport and a collaborative working relationship with WCHC.
- Assist a panel of clients to gain access to health-related social services (including identifying socio-economic issues) that affect patient overall health and wellbeing; will develop health/social management plans and goals with patients and act as a case manager for patients and their health and wellness team.
- Continuously expand knowledge and understanding of community resources/services; facilitate client access to, and assist client in utilizing those services, including scheduling appointments with social services agencies and assist with completion of applications and making phone calls for programs for which they may be eligible.
- Assist patients with eligibility for Medicaid/Oregon Health Plan (OHP) and work to increase OHP members particularly in local marginalized and underserved community populations. Assist patients with access to and eligibility for Marketplace Health Insurance programs
- Accompany the MHU to all events and activities and act as host for all participants, including Waterfall staff, patients, and community partners.

Education and Experience:
- Bachelor’s Degree required.
- Proficient in Microsoft Office.
- Experience with Electronic Health Record systems preferred with preference to EPIC users.
- Experience with data aggregation and display technologies including analytics platforms such as Excel and Tableau.
- Must possess the ability to work independently and as a member of a team with strong organizational, communication, and interpersonal skills to support multiple priorities at multiple locations.
- Strong verbal and written communication skills.
- Maintain confidentiality – in accordance with WCHC policies and HIPAA regulations.
Licensure/Certification Requirements:
- Oregon Driver’s License in good standing.
- Certified Traditional Health Worker credentials preferred or obtained within 1 year of employment, and recertify prior to expiration date, and
- Certified Oregon Health Plan and Marketplace Assister within 6 months of employment. Employer offers training.
- Current American Health Association (AHA) Basic Life Support Card (BLS) within 2 weeks of hire. Employer offers training.

- Prolonged periods sitting at a desk working at a computer.
- Able to climb steep steps, drive, park, and stabilize a large RV (MHU).
- Able to lift up to 50 pounds.
- Able to travel as needed.
