Position Summary:
In this role, you will provide hands-on nursing care to our patients, working closely with the interdisciplinary team to support individualized care plans, deliver symptom management, and foster a comforting environment for patients and their loved ones.

Key Responsibilities:

• Provide direct patient care, following established protocols and care plans.
• Administer medications and treatments as prescribed, ensuring accurate documentation.
• Monitor and report changes in patient conditions to the RN and medical team.
• Educate patients and families on symptom management, comfort measures, and available resources.
• Collaborate with the hospice care team to ensure seamless, compassionate care delivery.
• Maintain accurate, timely documentation of all patient interactions in accordance with regulatory requirements.

Qualifications:

• Current LPN license in Georgia
• Minimum of 2 years experience in hospice, palliative care, or a related field (preferred)
• Strong interpersonal and communication skills
• Ability to work independently and in a team-oriented environment
• Compassionate, patient-focused approach to care

Schedule:
This is a PRN (as-needed) position with flexible scheduling to meet patient and family needs.
