Job Description

The Health Care for the Homeless (HCH) area of Human Services and Public Health (HSPH) is hiring a Social Work Unit Supervisor. This position will be responsible for supervising a team of Mental Health - Substance Abuse Disorders staff in the HCH Program, a robust Federally Qualified Health Center that serves over 4,000 people experiencing homelessness in over 16,000 visits annually. We are looking for someone who has a strong clinical background and is skilled and experienced in working with individuals experiencing homelessness. This role will support the HSPH division's mission of "improving the health of our communities" and the goal of "assuring that all people's basic needs are met" and is a response to the growing unsheltered population in our community, and the rising needs for integrated mental health care, harm reduction services and the opioid crisis, and drug-related infectious disease outbreaks and syndemics.

Location and hours:
This position is hybrid and will be performed both on-site at Hennepin County community sites and remote as job duties require. Work hours will be Monday through Friday, 8 a.m. to 4:30 p.m. with an occasional weekday evening. No holidays or weekends. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In this position, you will:

 
  • Supervise Health Care for the Homeless MH-SUD and Outreach staff by establishing performance expectations, evaluating team and individual results and providing feedback and coaching, as appropriate. 
  • Manage HCH’s MH-SUD program and work with other HCH leaders to provide equitable integrated health to patients.
  • Lead and work with the HCH Social Service Team in following these objectives as related to the public health priorities of the opioid crisis and drug related infectious disease work:
    • Engage with patients who are sheltered, unsheltered or are adjacent to the unsheltered community who do not seek traditional methods of health care.
    • While building relationships and trust, promote harm reduction while identifying health needs, assess readiness to engage in care; assist with addressing patient identified needs. 
  • Serve as a liaison to other community MH-SUD agencies and Hennepin County departments including Public Health and Human Services Housing Stability/Homelessness to respond to MH-SUD programming needs and to participate with staff on broader Hennepin County projects.
  • Represent the department on committees and task forces, participate in strategic planning and resource development
  • Manage quality assurance and quality improvement efforts related to MH-SUD and social services.
  • Assist with grant management and program development across the HCH team

Need to have:

  • One of the following:
    • Master's degree or higher in social work or closely related field (or licensed as a graduate social worker by the Minnesota Board of Social Work) and three years or more of experience as a social worker.
    • Bachelor's degree in social work, psychology, sociology, or human services, and 27 quarter credits or more of graduate work in areas such as social work methods, human growth and behavior, principles of social welfare, and four years or more of supervised experience as a social worker.
    • Six years or more of experience as a Senior/Psychiatric/Child Protection Social Worker at Hennepin County.
  • A valid driver's license and daily access to reliable transportation to travel primarily within/outside of the metro area to meet with clients in their residential settings and/or perform authorized off-site work duties.

Nice to have:

  • Minnesota certification(s)/licensure(s) in one or more of the following:   Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage & Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC) and/or Licensed Psychologist (LP).  
  • Experience: 
    • With supervisory or lead work.
    • Working with homelessness and those experiencing housing instability.
    • Working with electronic health record documentation.
    • Addressing racial equity, gender equity, LGBTQIA equity, neurodiversity equity, and/or differing ability equity, either professionally or through education.
  • Knowledge of: 
    • Cultural differences in beliefs and practices relating to mental health, treatment, family structure, and dynamics.
    • Government and community mental health response systems and resources.
    • Ethics, rules, and statutes related to mental health services.
  • Ability to:
    • Function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment.
    • Effectively interact with people from diverse backgrounds and/or populations who experience health disparities.
    • Communicate effectively with a variety of stakeholders including other emergency responders such as Emergency Medical Services (EMS) and law enforcement.
